Paxlovid is a combination antiviral consisting of two tablets. One tablet was Nirmatrelvir (150mg), an inhibitor of the 3CL protease (also known as the main protease, Mpro) of the novel coronavirus. 3CL protease is a key protease that hydrolyzes RNA-encoded polyprecursor protein to produce functional protein during the replication of coronavirus. Nematavir can prevent the replication of SARS-CoV-2 coronavirus by inhibiting 3CL protease. The other tablet is Ritonavir (100mg), an HIV-1 protease inhibitor and CYP3A4 (cytochrome P4503A4 enzyme) inhibitor. Although Ritonavir is not active against SARS-CoV-2Mpro, it inhibits CYP3A4-mediated metabolism of nematavir to help it remain active in the body longer at higher concentrations.
