Foralumab (development code NI-0401) is an investigational, fully human anti-CD3 monoclonal antibody being developed for the treatment of autoimmune and inflammatory diseases. Unlike earlier murine anti-CD3 antibodies that caused significant side effects, foralumab is engineered as a non-FcR binding, non-mitogenic IgG antibody. It functions as a T-cell modulator rather than a depleting agent. By binding to the CD3 epsilon chain of the T-cell receptor (TCR) complex, it delivers a weak signal that can induce immunomodulatory effects, such as the generation of regulatory T cells (Tregs) and the downregulation of pro-inflammatory T-cell responses, without causing widespread T-cell activation or cytokine release syndrome. A unique feature of its development is its formulation for intranasal administration, aiming to induce local mucosal immunomodulation with minimal systemic exposure for diseases like multiple sclerosis (MS) and COVID-19-related inflammation.
